Botox insurance provides coverage for the unique risks and liabilities associated with Botox & Dermal Fillers injected into many parts of the human anatomy. Our policies include professional & general liability for the injection of Botox, Dysport, Xeomin, Jeuveau, Daxxify or any FDA similarly approved products including treatments for Hyperhidrosis, & treatments done in the Masseters, Decolletage, Trapezius and / or Platysmal Bands. Coverage also applies for Dermal Fillers done in the Face, Earlobes, Décolletage, Neck, Hands, Buttocks, Legs or Upper Arms.

Yes, at PPIB we understand that every business is unique and has distinct insurance requirements. We offer customizable Botox & Filler policies to endure that your coverage aligns with your specific business needs.

Yes, they can be. If they are your employees, you should have them covered under your policy and we can protect you from claims that arise from their clientele. If they are independent contractors who are working under your business name then we can also include them. If they are working under a different business name then they will need to obtain their own coverage and name you as an additional insured.

If you are offering these services in a state that allows these services to be performed without a Doctor/Nursing License, we are able to cover anyone who can legally perform the service, provided that they have the proper training and are obeying all of the laws & regulations set by their state Board of Health.

Yes, at any time during your policy term, you can contact PPIB and request an endorsement. Additional forms and/payment may be required.

Yes, Botox insurance policies typically have exclusions or limitation that outline specific situation or circumstances where coverage may not apply or may be limited. These exclusions and limitation can vary depending on the service being provided. Some of the main Exclusions are:

  1. Injectable Products not purchased from an approved United States and/or Canadian wholesaler/retailer
  2. The use, administration or application of Botox to any person who has a neuromuscular disease
  3. The use, administration or application of Botox contrary to manufacturer’s guidelines
  4. Work on veins over 3.0 millimeters
  5. The use of any products known to be unsafe or banned by the FDA
  6. Any fat grafting procedure
  7. Off-Label Botox treatments other than the treatment of the glabella, brow / forehead, mouth / lip area, chin, under arms, bunny lines, crows-feet, the treatment of migraines, hyperhidrosis, masseters, décolletage, trapezius and platysmal bands
  8. The use, administration or application of any Dermal Filler in any area other than the following: face, décolletage, neck, hands, earlobes, tear troughs, buttocks, legs or upper arms.
  9. Use of more than 2 vials of Botox in the trapezius
  10. The use of more than 40 vials of Dermal Filler in the Buttocks per session; the use of more than 12 vials of Dermal Filler in the Legs per session; the use of more than 6 vials of Dermal Filler in the Upper Arms per session
  11. Injections into or in close proximity (1-1.5 cm) to the armpit region or lymph nodes
  12. Injections unto or within 1.5cm of salivary glands, lymph nodes, muscles in the neck

The use of consent and medical history forms are required for all Botox & Filler services.We can provide pre-approved consent forms if you do not have your own. If you wish to use forms you have created we will require you to provide copies to our office for review.

Please check with your state requirements as to what the statute of limitations are for your service. Statutes of limitations are usually a minimum of 2 years with some states going to 6 or more years.
