The use of consent and aftercare forms are required for all IV Therapy services. We can provide pre-approved consent forms if you do not have your own. If you wish to use forms you have created we will require you to provide copies to our office for review.

Please check with your state requirements as to what the statute of limitations are for your service. Statutes of limitations are usually a minimum of 2 years with some states going to 6 or more years.
